For Username or Password changes select one of the following options:
Lookup PVT Username (Must know SSN)
Change PVT Password (Must know your old password)
Reset PVT password (Must know SSN and Birthdate (mmddyy) numbers)

Note that password information cannot be given out over the phone or sent to a non-Columbia State email account.
